A daycare sign-in sheet is a form that parents fill out when dropping their children off at daycare, preschool, afterschool, or other child care programs. They sign it again when they pick their children up. This provides an official record of when the child was in the care program.

You can customize this sign-in sheet to match your daycare’s branding.
Click on the name “Morning Glory Daycare” to enter in the name of your childcare center. If you need a space for a parent or guardian signature, you can change the Notes column to read Signature. If you want to collect some other information (allergies, medication administration, etc.) you can change this column to fit your needs.
